If you wish to get involved in local health and social care monitoring and development contact the new Local Involvement Network (LINk) They will be drawing people and groups together to focus on specific areas of conern or need in the community.
Lewes ComNet and Wealden ComNet help link groups into issues related to the Choosing Health priorities such as healthy eating, mental health and wellbeing and substance misuse.
LTC Self Management / Patient Education Options Review
Nicky McCrudden, Long Term Conditions Projects Consultant, is currently working on a project exploring the provision of services to help adults with Long Term Health Conditions to be more actively involved with self-management and self-care.
The PCTs have recently produced a paper reviewing the evidence for a number of different ways of supporting self-care, and exploring options for potential services for East Sussex . Nicky is currently looking to get the views of a number of interested parties on the options.
Please find attached the paper reviewing the evidence and the 4 potential options for service delivery across East Sussex . Also attached is a questionnaire related to the paper and the options.
